There is a signal, but it is not a simple cure
A 2023 review of cardiovascular trials reported favorable intima-media thickness and some plaque-related measures with berberine added to statins. It also found no monotherapy advantage for the Crouse plaque score and highlighted limitations in the underlying research quality. These mixed findings do not establish a dependable supplement-only regression strategy. [1]
Keep the endpoint clear
Cholesterol, arterial-wall thickness, plaque scores and clinical events are different outcomes. A change in one marker cannot show that an artery is unobstructed or that future heart attacks are prevented. Heterogeneous adjunctive studies also cannot isolate the same benefit for every formulation. [1]
Treat the established risk factors
NHLBI recommends heart-healthy habits and appropriate medicines to manage cholesterol, blood pressure, diabetes and other risks. Statins and other therapies are chosen for the individual. Do not replace them with berberine or add a supplement without checking interactions. [2][3]
A lower number is not an image of the artery
Imagine LDL falls after a supplement is added to a statin. That does not measure plaque volume or show which treatment caused the change. Interpret results with the cardiovascular plan and avoid declaring a blockage reversed without relevant assessment. [2][1]
| Outcome | Not interchangeable with |
|---|---|
| LDL cholesterol | Plaque volume |
| Arterial-wall thickness | An unobstructed artery |
| Plaque score change | Fewer heart attacks |
| Adjunctive trial signal | Reliable stand-alone treatment |
