Low ferritin: food, iron replacement and finding the cause

Low ferritin can indicate depleted iron stores, but the treatment plan needs the full blood results and the reason iron is low. Food can improve intake; confirmed deficiency may also need iron replacement and follow-up.

Ferritin is an iron-storage marker

MedlinePlus explains that ferritin binds and stores iron. The test is often interpreted alongside other iron tests and the blood count. Hemoglobin measures a different aspect of blood, so a ferritin result should not be treated as interchangeable with the hemoglobin number. [1][2]

Ask why stores became depleted

Heavy menstrual bleeding, gastrointestinal blood loss, pregnancy and inadequate intake can contribute to iron deficiency. Absorption problems are also relevant. Replacing iron without addressing ongoing loss can leave the underlying problem unresolved. [3][1]

Food and replacement have complementary roles

Meat and seafood provide heme iron; beans, lentils and fortified foods provide nonheme iron. Vitamin C can improve absorption of nonheme iron. A clinician may recommend an iron preparation when deficiency is established, selecting the dose and schedule around severity, tolerance and the cause. [4]

Follow the response rather than a universal target

Iron treatment and rebuilding stores can take months, with repeat testing guiding the course. Discuss constipation, nausea or other side effects rather than abandoning treatment or multiplying the dose. Keep iron securely away from children because overdose can be dangerous. [3][4]

Bring the result and symptoms to the review

Imagine ferritin is flagged low but hemoglobin is in range. Ask whether this represents iron depletion, whether a source of loss needs investigation and when to retest. Do not assume that a normal hemoglobin makes the storage result irrelevant or that every fatigue symptom must have the same cause. [1][2]
