Are probiotics safe during pregnancy?

Some studied probiotics have reassuring pregnancy safety data, but there is no blanket assurance for every strain, product or purpose. A gestational-diabetes prevention review also identified a pre-eclampsia safety signal. Review the exact product and reason for use with the maternity team rather than choosing a supplement solely because it is marketed as natural.

Reassuring findings have limits

A 2021 systematic review found no serious adverse effects attributed to the studied interventions, with reported effects mainly digestive. However, adverse-event reporting was incomplete, products and timing varied, and some studies had risk-of-bias concerns. This cannot certify every retail supplement or high-risk pregnancy. [1]

One important review found a different safety concern

Cochrane’s 2021 review of probiotics for preventing gestational diabetes found uncertain preventive benefit and increased pre-eclampsia in the included trials. That specific result should not be generalized into certainty that all probiotics cause harm, but it argues against routine use for that goal without clinical review. [2]

There is no universal safe-strain shopping list

Ask whether the exact strain, amount, additional ingredients and intended outcome were studied in a relevant population. Pregnancy stage, medical conditions and other treatments matter. A food containing cultures, an oral capsule and a vaginal product are different exposures. [3][1]

Make the decision specific

Imagine someone wants a probiotic to prevent diabetes despite normal screening. The maternity team should review that purpose and the safety evidence rather than relying on general gut-health marketing. A different indication may involve a different benefit-risk assessment. [2]
