They act in different treatment contexts
NHS describes trazodone as a treatment for depression and anxiety with possible sleepiness. Melatonin is a hormone involved in circadian timing and has selected sleep-related uses. Sedation is not the same as correcting a circadian problem or providing effective long-term insomnia treatment. [1][2]
Guidelines do not identify either as the default winner
The 2026 AASM combination guideline reviews the earlier conditional recommendations against trazodone and melatonin for routine chronic-insomnia pharmacotherapy because of limited evidence and other considerations. It emphasizes CBT-I and individualized decisions about adding medication. These recommendations do not mean either medicine has no role in any patient or diagnosis. [3]
Compare harms and the reason for prescribing
Trazodone can cause dizziness and daytime sleepiness and should not be stopped abruptly without a prescriber’s plan. Melatonin can also impair alertness, interact with medicines and has limited long-term safety evidence. There is no valid milligram-for-milligram conversion between them. [1][2]
Make a goal-based choice
Imagine trazodone is treating depression as well as sleep symptoms. Replacing it with melatonin could leave the underlying treatment need unmet. Discuss whether the problem is chronic insomnia, a timing disorder or another condition, and use that assessment to select care. [1][3]
| Feature | Trazodone versus melatonin |
|---|---|
| Main role | Antidepressant versus circadian hormone |
| Chronic insomnia | Neither a universal first choice |
| Adverse effects | Both can impair alertness |
| Switching | Needs an individualized plan |
