What the experiment compared

The trial enrolled 184 people with fatty liver and compared lifestyle intervention alone, lifestyle plus pioglitazone and lifestyle plus berberine. Treatment assignment was open-label. [1]

Liver-fat content declined more with berberine plus lifestyle than with lifestyle alone. Some weight and metabolic measures also favored the combination. Related animal experiments explored mechanisms separately. [1]

Interpretation and limits

The authors declared no competing interests; funding was not established from the abstract. The short study and open-label design limit inference, and animal gene-expression findings are not human mechanistic proof. Liver-fat reduction is not equivalent to demonstrated fibrosis reversal or improved survival. [1]

A hypothetical distinction

Imagine an imaging measure improves after a combined intervention. That finding does not establish every later disease outcome or show what happens without the shared lifestyle program.
