A high ranking is not proof of superiority
The 2025 network meta-analysis combined thirty trials of several SIBO treatments. Berberine ranked highly, but its comparison with rifaximin was not statistically significant. A ranking statistic is neither an individual chance of cure nor proof that the highest-ranked treatment is best for every patient. [1]
The review does not establish a standard dose
Regimens and diagnostic methods varied; some comparisons relied on limited samples and many studies were open-label. The authors characterized the findings as hypothesis-generating and did not formally assess comparative safety. Copying one study schedule would therefore give a false impression of a validated universal regimen. [1]
Confirm the condition and address recurrence
SIBO symptoms overlap with other digestive disorders, and breath tests have false positives and negatives. The Brazilian gastroenterology position paper emphasizes appropriate diagnosis, antibiotic treatment and attention to underlying factors and recurrence. Bloating alone is not enough to choose an antimicrobial course. [2]
Review a proposed adjunct with gastroenterology
Berberine itself can cause digestive symptoms and interact with medicines. Imagine symptoms improve during an unsupervised course but quickly return. That is a reason to revisit the diagnosis and drivers, not automatically repeat or escalate the dose. [3][2]
| Evidence | Limit |
|---|---|
| High network ranking | Not proven superiority |
| One study schedule | Not a universal dose |
| Less bloating | Not proof of eradication |
| Recurring symptoms | Reassess causes and diagnosis |
