Does creatine lower cortisol? A training supplement is not a cortisol treatment

The inspected evidence does not establish creatine as a dependable way to lower cortisol or treat an endocrine disorder. Exercise-related hormone measurements, subjective stress and sustained pathological cortisol excess are different outcomes.

Cortisol has necessary functions

NIDDK describes cortisol as supporting the response to stress, blood pressure, blood glucose and metabolism. It is not a toxin that should always be suppressed. Sustained abnormal excess is a medical question, not a diagnosis based solely on feeling stressed or having a difficult workout. [1]

The combination research is limited

A 2019 systematic review of creatine plus HMB included six studies, but only one assessed the relevant hormones. That study did not find a cortisol difference after six weeks. The review reported no funding or conflicts, and its heterogeneous short studies do not establish a universal hormone effect. [2]

Do not transfer an ingredient or performance result

A result for HMB alone cannot automatically be assigned to creatine, and improved strength does not demonstrate lower cortisol. Likewise, preliminary depression research does not establish that creatine normalizes stress hormones in otherwise healthy people. [2][3][4]

Testing depends on the clinical question

For suspected Cushing syndrome, clinicians use the history and appropriately timed blood, saliva or urine tests, often with confirmation. Symptoms overlap with other conditions. Do not self-treat an isolated cortisol result with creatine or change prescribed glucocorticoids without medical advice. [1]

Ask what the claim actually measured

Imagine a product promises lower stress hormones because users report better workouts. That is not a cortisol measurement, and even one exercise-related reading would not establish treatment of sustained hormone excess. [4][1]
