Melatonin for tinnitus: one trial does not identify the best dose

A small crossover trial found improvement in some tinnitus measures with melatonin. Its single tested regimen cannot establish the optimal dose or lasting benefit for an individual.

What the study measured

Adults with chronic tinnitus received melatonin or placebo for 30 days, followed by a washout and the opposite treatment. Sixty-one participants completed the randomized, double-blind study. [1]

The melatonin condition produced greater reductions in tinnitus matching and self-rated tinnitus measures than placebo. The authors also reported improved sleep and associations between baseline characteristics and response. [1]

How far the finding goes

Testing one nightly amount does not compare several doses or identify an optimum. Subgroup associations in a small trial also need confirmation before being used to select likely responders. Funding and conflicts were not established from the abstract. [1]

The findings are limited to the study’s short treatment periods and chronic-tinnitus population. They do not establish a cure, permanent improvement or an explanation for a new ear symptom. A research regimen is not automatically a personal treatment instruction. [1]

A hypothetical distinction

Imagine a trial tests one amount and sees an average improvement. Without comparing other amounts, it cannot show that the tested amount is the smallest effective dose or the best choice for every participant.
