Iron for restless legs syndrome: diagnosis and iron studies come first

The 2025 AASM guideline includes specific iron treatments for selected people with restless legs syndrome. It does not recommend treating every nighttime leg symptom with iron or magnesium.

Restless legs syndrome is a clinical condition

AASM advises regular iron studies, including ferritin and transferrin saturation, in clinically significant restless legs syndrome. The results influence whether oral or intravenous iron is considered. The iron criteria used in this setting differ from ordinary population reference ranges. [1]

The guideline identifies its numerical iron thresholds as consensus guidance that has not itself been empirically tested. A result marked normal by a laboratory therefore does not independently settle the treatment question, and a symptom alone does not establish iron deficiency. [1]

Recommendations are specific to the preparation and population

For adults with appropriate iron status, AASM strongly recommends intravenous ferric carboxymaltose over no such treatment, with moderate certainty of evidence. It conditionally suggests oral ferrous sulfate, also with moderate certainty. These recommendations are not evidence that all iron preparations, or magnesium, are interchangeable. [1]

Treatment selection requires benefit–harm assessment. The guideline notes risks including low phosphate with ferric carboxymaltose. An infusion should not be chosen only because its recommendation is stronger without considering the individual setting. [1]

Iron is one part of the evaluation

AASM also advises addressing factors that can worsen symptoms, including certain medicines, alcohol, caffeine and untreated obstructive sleep apnea. Do not stop prescribed medicines independently; review them with the treating clinician. [1]

Keep the diagnosis attached to the evidence

Imagine someone has leg discomfort at night but has not been assessed for restless legs syndrome. A trial in diagnosed RLS cannot determine the cause of that person’s symptoms. Evaluation comes before applying its treatment findings.
