The study and its comparison
PROPATRIA randomized 298 patients with predicted severe acute pancreatitis to a multispecies probiotic preparation or placebo, administered enterally. After two diagnostic exclusions, 296 were analyzed. [1]
Infectious complications occurred in thirty versus twenty-eight percent, without a significant benefit. Twenty-four probiotic recipients died versus nine controls. Bowel ischemia occurred in nine probiotic recipients and no controls; eight of those cases were fatal. [1]
What the finding establishes
This combination, route and severe-disease setting cannot be generalized into a numerical risk for healthy users. It does establish that a prevention hypothesis can have serious unexpected consequences. Funding and conflicts were not established from the abstract. The study’s harms concern bowel ischemia and mortality, not proof that every episode of bleeding after a probiotic has that cause. [1]
A hypothetical distinction
Imagine an infection-prevention strategy leaves infections unchanged but increases deaths. Safety outcomes must remain central.
